    Responsibilities
    
    
    • C oordinate and advocate for our patients' care & create an appropriate discharge plan.
    • Review patients' charts, and meet with families and healthcare providers to establish the most suitable course of action.
    • Should remain in contact with patients and their families during the hospitalization stay.
      Qualifications
      
      Job Requirements:
      • Current RN license in the state of Texas
      • Registered Nurse with an Associates, Bachelor's or Master's Degree
      • Minimum of three (3) years of clinical experience Preferred Job Requirements:
        • Case Management certification
        • Experience in utilization review, discharge planning, quality programs by Jobble
